Lewis, 4530 Tyrone St., Sherman Oaks, was pronounced dead on arrival at Valley Receiving Hospital after he slipped while playing at a construction site. Rescuers, above, treated the 9-year-old boy at the scene of the accident, 14640 Dickens St., but it was too late.
