"On House. (For One Year.)"--text, top center of image. "No. 5575. Whereas, John Conrad hath paid to the President and Directors... Common Seal hereunto affixed on the Twenty first Day of February in the Year of our Lord One Thousand Eight Hundred and nine. N.B. This Policy may be transferred by indorsement [sic]..."--text, center of image. Handwritten manuscript text on verso.
