The form has three columns that need to be filled in and dead weight calculated; in the upper-right hand corner is space for the date, hour, and condition of loading; in the upper left hand side is space for the voyage number. Form is hole-punched along left margin. Focus of item: Mariposa (1956-) ; Monterey.
