Written in Arequipa, Peru, dated 7 February 1571. The front pastedown bears the inscriptions "N__?___miermano" and "No 26"; on the back pastedown a pressmark "A.10" in pencil. On f. v a tipped-in note from Madrid, dated 30 January 1742, stating that the manuscript had been purchased from the lawyer Joseph Antonio de Ruedas with the help of Joseph de Silva.
