Photograph caption dated August 20, 1964 reads, "Valley Times photographer Steve Young captures classic delivery of Sandy Koufax scheduled to pitch against Milwaukee Friday night at Dodger Stadium.” Photograph article partially reads, “Details on the exact condition and causes of Koufax’ ailing arm are not clear. As long ago as April 22, he was forced to quit after one inning in St. Louis. He was quickly flown home after examination by Cardinal team physician I. C. Middleman, who said Koufax suffered from a muscle spasm in the upper forearm and tenderness in the elbow. Following extensive cortisone treatments here, he returned in two weeks and pitched 10 innings to beat the Cubs, 2-1, for his second win of the season.”
