Image of a discharge certificate form with an American bald eagle emblem with wings displayed standing on a Union shield with American flags and an olive branch below the title; blank "Oath of Identity" form on verso.
Form is filled in manuscript for William Lair, a private in Company A, 2nd Regiment of Artillery Illinois, who was discharged in Carrollton, Louisiana, on December 31, 1863; signed at bottom by commanding officer, Captain Herman Borries. "[A. G. O. No. 99]"--text, bottom left margin.
